Sever's disease (calcaneal apophysitis) is not actually a disease. It is one of the most common causes of heel pain in active children and adolescents, particularly those participating in running and jumping sports. It is a load-related condition that occurs while the heel growth plate is still developing.

During childhood and adolescence, the heel growth plate (calcaneal apophysis) is still developing and is predominantly made of cartilage rather than mature bone. This makes it less tolerant of the repeated pulling forces from the Achilles tendon and the impact forces generated during running and jumping activities.

When these loads exceed the tissue's capacity, the growth plate can become irritated, resulting in pain and swelling. During periods of rapid growth or “growth spurts” this can further decrease the capacity at the growth plate and make pain flares more likely.

  • Spikes in training load or activity levels.  
  • Repetitive running, jumping and explosive sports.  
  • Participation in multiple sports with limited recovery.  
  • Reduced calf muscle strength or lower limb strength/capacity.

Treatment and management options

Treatment focuses on reducing irritation to the growth plate while helping children remain as active as possible. Following a thorough assessment, your physiotherapist will develop an individualised treatment plan based on your child's symptoms, activity levels and sporting goals.

Treatment may include:

  • Load management strategies.  
  • Individualised rehabilitation exercises.  
  • Taping techniques to reduce load through the Achilles tendon.  
  • Manual therapy where appropriate.  
  • Footwear assessment and advice.  
  • Education and guidance for a safe return to sport.
