PHYSIOTHERAPY MANAGEMENT OF ARTHRITIS

Strong evidence confirms the effectiveness of physiotherapy to treat arthritis and musculoskeletal conditions, with interventions to control pain and improve physical function.

Backed by some strong evidence from randomised controlled trials and systematic reviews, the treatment of arthritis and musculoskeletal conditions is a core function of physiotherapy practice. Timely access to physiotherapy services for patients with arthritis-related problems can delay disease progression, reduce relapse and disease severity, and assist in prevention of further problems.

Appropriate exercise plays a vital role in prevention and early intervention, and several studies demonstrate the effectiveness of land-based therapeutic exercise in managing both acute and chronic arthritis and related conditions, Research shows that specific exercise keeps joints mobile, improving cartilage health while maintaining muscle strength.

The benefits of exercising in water are also well documented, with good evidence supporting the role of aquatic physiotherapy in reducing pain and improving joint mobility, strength and balance, especially among older people and those with rheumatic conditions and chronic lower back pain.

How do you measure up?

Most Australians are well aware that being overweight or obese is not good for your health. We know that obesity reduces life expectancy and is a major risk factor for coronary heart disease. Obesity also reduces quality of life due a range of associated conditions including musculoskeletal difficulties, osteoarthritis, type 2 diabetes, high blood pressure, deep vein thrombosis, gout and infertility. But how does being overweight or obese affect your likelihood of developing cancer and how do you know if you are at increased risk?

Recent findings by the Cancer Council Victoria reveal that there is now a very simple measure that everyone can perform to determine your risk of developing certain types of cancer. A simple measurement of waist circumference is all you need. The research shows that a waistline of over 100cm for men, and 85cm for women, significantly increases the risk of cancers such as:

  • Colon cancer
  • Some cancers of the oesophegus
  • Aggressive prostate cancer
  • Cancer of the uterus
  • Rectal cancer
  • Kidney cancer
  • Breast cancer

It was found that for women with a waist circumference over 85cm, there was a 22% higher risk for breast cancer, and a 33% higher risk for colon cancer. For men whose waistline measured over 100cm, there was a 72% increased risk for colon cancer and 43% increased risk for prostate cancer.

It is clear that obesity causes cancer but the reasons why are less clear. The link is possibly related to the high levels of insulin measured in obese patients which may directly or indirectly cause cells to grow and divide. Obesity also alters the levels of some hormones such as oestrogen, which may also cause cells to grow and divide, resulting in cancer.

The good news is that you can reduce your cancer risk if you reduce your waistline.

Measuring your waistline is easy:

  1. Measure around your waist half way between your lowest rib and the top of your hip bone, this will be about level with your belly button.
  2. Best to measure against the skin, not through clothing.
  3. Measure after breathing out normally.
  4. Pull the tape measure taut but do not compress the skin.

What can I do if my waist measurement too large?

Current evidence suggests that the total energy people eat (calories or kilojoules), combined with their level of physical activity, is most important in determining their cancer risk.

The solution is simple: eat less and move more!

Aim to do at least 30 minutes of moderate level continuous activity such as walking, 5 to 6 times a week.
